Medical Records FAQs - Winter Haven Hospital

Who can sign the Release of Information form?

Any of the following individuals can sign the Release of Information form:

  • Patient - 18 years of age or older
  • Medical Power of Attorney (must state it includes medical information)
  • Healthcare Surrogate
  • Legal next of kin

Can I get all of my records from the Medical Records Department?

Yes. Please call (863) 293-1121 ext. 1169 for more information.

How long does the hospital keep my records?

We keep your medical records for 10 years. Health Information Services processes all requests for Release of Information. Call (863) 293-1121 ext. 1169 for more information.

How long will it take you to process my request?

We generally take 4-5 business days to fulfill your medical records request.

Can you send my medical records to my doctor for me?

Yes, we will be happy to fax or mail your records to your doctor's office or treatment center. Please indicate this information on the Release of Information Request form.

Is there a charge for copying my medical records?

There is no charge for medical records if we send the records to your doctors. However, if you request your records be handed to you, then the charge is $1 per page.

Can I request a copy of a birth certificate from you?
