Discharge and Medical Records
When it’s time to go home, your child's physician will determine when he or she is able to leave the hospital. Your child's nurse will give you printed discharge instructions and any prescriptions needed for recovery. Discharge time and instructions are personalized to each particular procedure and how well your child is recovering. Please note that prescriptions need to be filled at an outside pharmacy and should be filled right away to ensure your child’s comfort and recovery.
If you have questions or concerns about the discharge process, please speak with your nurse or social worker.
At some point in the future, you may want copies of your child’s medical records. To request a medical record, you can download our printable Release of Information Form. Please print and fill out the form, sign it and mail it to the hospital where your child was treated. See list of hospital locations.

Please note: We must have your signature to release the records you request. To protect your confidentiality, all patients 18 years of age or older must sign the Release of Information Form. A parent or a legal guardian may sign for children under the age of 18.
