Pre-Surgical Assessment Process

Thank you for choosing BayCare Surgery Centers for your upcoming surgery or procedure.

To provide you with excellent care, we ask that you complete your health history (also known as a “medical passport”) through our secure online portal as soon as your surgery has been scheduled. When you are ready, choose the location below where your surgery is scheduled to be performed and you will be redirected to One Medical Passport (One Mnet Health) to complete your medical history.

You may receive a text or email reminder from One Mnet Health to complete your medical passport. The links within the message(s) will also direct you to the online portal. Once you do this, our pre-admission nurses can begin organizing and documenting your complete medical history to prepare for your surgery.

How to Complete Your Medical Passport Online

Be sure to have the following information available before starting:

• Your health insurance information and government approved photo identification
• The name of the physician who is performing your upcoming procedure
• The names, addresses and phone numbers of your physicians
• A list of all medications you are taking, their dosage and frequency, including over-the-counter items
• A list of all surgical procedures you have had

New Users:

Click 'Register' to create your first Medical Passport.

1. Enter your demographic information to create your account.
2. Answer the questions to complete your online health history.

Existing Users:

If you've previously created a Medical Passport Account, you will need to log in to update your health history.

1. Enter your original username in the 'Welcome Back' area.
2. Once you've successfully logged back into your account, click on the button located on the top right of the page.

Forgot Username and/or Password?

• Click 'Sign in Help' if you don't remember the username you created upon initial registration.
• Click *'Forgot Password?' if you don't remember your password and we will guide you through an automated reset process.

*The 'Forgot password' link will only show after you have entered an existing username.
