BayCare, the leading not-for-profit health care system in West Central Florida, is calling on residents of Hillsborough, Manatee, Pasco, Pinellas and Polk counties to participate in the 2025 Community Health Needs Assessment (CHNA) survey. The anonymous survey gathers insights from local individuals to identify and address pressing health concerns in the region. To take the survey, use this link or scan the QR code below with your smartphone camera. The survey closes Feb. 14, 2025.

Hurricanes Helene and Milton, along with the busy holiday season, understandably diverted attention away from the survey that launched last September. BayCare acknowledges these disruptions and emphasizes the importance of community input to shape future health initiatives.

Why Your Participation Matters
The CHNA survey is a vital tool that enables BayCare to develop programs tailored to community needs. The survey covers topics like access to health care, mental health, substance use, food insecurity and other social determinants of health. Previous assessments have led to impactful initiatives, including:

  • Health Navigation Services: Connecting community members to essential resources through platforms like FindHelpFL.
  • Behavioral Health Services: Expanding mental health support through initiatives like mental health first aid courses for first responders, educators and public officials.
  • Addressing Food Insecurity: Launching programs like Community Food Clinics and school-based food pantries in partnership with Feeding Tampa Bay.
These efforts have significantly improved health outcomes in the greater Tampa Bay area. An 18-month progress report published in August 2024 highlighted that BayCare's community benefit initiatives led to over half a million engagements or interventions, demonstrating the impact of community feedback.
