The Heart Function Clinic is located on the second floor of the Medical Arts Building at St. Joseph’s Hospital. Our physicians care for patients who have a new heart failure diagnosis or have frequent admissions to the hospital due to heart failure. Our mission is to provide high-quality, comprehensive care that improves quality of life and reduces hospital admissions and readmissions. Patients and families who visit the clinic receive special, individualized care.

The Congestive Heart Failure and Heart Function Clinic offers a collaborative management program focused on slowing disease progression. We work closely with our cardiovascular surgical team to deliver advanced heart failure therapies, helping patients lead high-quality lives. The clinic is a resource for patients and families who:

  • Are being discharged from the hospital (we aim to see patients within three to seven days after discharge).
  • Are at high risk for admission/readmission to the hospital.
  • Are newly diagnosed with heart failure.
  • Need close follow-up to optimize their medication regimen.
