The Heart Function Clinic is in Suite 250 on the Morton Plant Hospital campus. Our physicians care for patients who have a new heart failure diagnosis or have frequent admissions to the hospital due to heart failure. Our mission is to provide high-quality, comprehensive care that improves quality of life and reduces hospital admissions and readmissions. Patients and families who visit the clinic receive special, individualized care.
At the Congestive Heart Failure and Heart Function Clinic, we offer a collaborative management program aimed at slowing disease progression. We work closely with our cardiovascular surgical team to provide advanced therapies, helping patients lead high-quality lives. The clinic is a resource for patients and families who:
- Are being discharged from the hospital (we aim to see patients within three to seven days after discharge).
- Are at high risk for admission/readmission to the hospital.
- Are newly diagnosed with heart failure.
- Need close follow-up to optimize their medication regimen.
