DOT Physical

Getting your DOT physical is easy with BayCare Urgent Care.  

Avoid a wait and schedule your DOT (Department of Transportation) physical at any of our convenient locations by using our online Save Your Spot appointment scheduler. You can also walk in to any BayCare Urgent Care for the physical.

Your physical will be done by a licensed BayCare health care provider listed with the Federal Motor Carrier Safety Administration (FMCSA) National Registry.

If your provider finds you qualified to drive a commercial motor vehicle (CVM), we’ll complete your Medical Examination Report for Commercial Driver Fitness Determination form, provide a summary and complete a Medical Examiner’s Certificate. Your DOT physical is valid for up to two years.  A Medical Examiner’s Certificate may also be issued for less than two years for a condition that is being monitored such as diabetes or high blood pressure.

With your permission, a summary of your visit can be sent to your primary care doctor.  

Important Things to Note about Your DOT Physical

  • Complete the driver information and health history portions on the Medical Examination Report for Commercial Driver Fitness Determination form before arriving for your physical
  • Bring copies of your immunization records. Our providers can validate immunizations only with sufficient documentation or immunizations performed at a BayCare Urgent Care. 
  • A drug test by urine sample will be taken to detect marijuana, cocaine, opiates and other drugs. An additional fee is required for a drug test. A positive drug test means that the Medical Examiner’s Certificate will not be issued and you could lose your commercial driver’s license. A refusal to take a drug test is considered the same as a positive drug test.
  • Insurance does not pay for this physical
