Case Management Services

Bringing a baby into the world and becoming a parent can be overwhelming even under the best of circumstances. Some new parents may be anxious or scared. Others may be concerned about financial matters or need assistance with transportation or child care.

We understand the many emotional and social needs you may have at this time, and want you to know that Morton Plant Mease Hospitals provides Case Management Services you may find helpful.

The Case Management Services professional can provide you with an information and resource guide that covers a wide variety of issues, such as:

  • Parenting classes, support groups, counseling
  • Birth certificates
  • Social Security
    Daycare and other school programs
  • Financial information
  • Food resources
  • Housing
  • Infant supplies
  • Out-patient medical care
  • Emergency assistance
  • Transportation

Healthy Start Program

The Healthy Start Program screening can determine your need for services. This program offers:

  • Education, counseling and support during labor and birth
  • Breastfeeding education
  • Help and encouragement

Florida's Healthy Baby Program

If you need more support, you can request visits from a community health nurse or perinatal support worker (social services coordinator). Transportation to and from appointments, educational classes and other social services are also offered.
