I reviewed the clinical notes by this provider. I am aware that clinical notes are now auto-populated, but this provider did NOT do any physical exam on me, so the notes were inaccurate.

I always have the best care and response from the office staff. One issue was contacting the provider by phone. The office telephone system would not allow me to leave a message or speak to someone when trying to schedule this appointment. I used the online portal mail and received outstanding support in getting my appointment scheduled.
