BayCare is a leading not-for-profit, community-owned academic health system committed to supporting the health and well-being of the Angeline community. As part of that commitment, you have access to a connected network of care, wellness resources and a health care navigator. Together, these services help you access and coordinate care, find answers to health questions and stay connected to the support you need close to home.

Health Care Navigator

The BayCare health care navigator, a registered nurse, provides free, one-on-one guidance to help you access the care you need. This includes help scheduling imaging and lab services, as well as BayCare Medical Group primary care and specialist appointments. The navigator can help address health concerns, connect you with the right care, provide follow-up support and education about the BayCare patient portal, the BayCareAnywhere® app and other digital health tools. In addition, the navigator supports ongoing health management through in-person blood pressure and diabetes screenings.
  • What is the BayCare health care navigator program?
    The BayCare health care navigator program helps Angeline residents access BayCare services, wellness resources and online health tools. Through the program, residents have a dedicated BayCare contact who can answer questions, provide guidance and help make the health care experience easier to navigate. 
  • Why is BayCare bringing a health care navigator to Angeline?
    BayCare and Metro Development Group share a commitment to supporting the health and well-being of Angeline residents. The health care navigator program was created to provide residents with convenient access to health information, resources and support within the community. 
  • What does support from a health care navigator look like?
    Everyone's needs are different. That's why we provide personalized support based on what's right for you. Whether you need help finding a primary care provider, coordinating appointments across multiple services, finding health resources or knowing where to go for care, your health care navigator is here to help. They provide guidance, coordination and follow-up to help you connect with the care and resources you need.
  • Does it cost anything to use the service?
    Included with living at Angeline. Costs for health care services and insurance coverage still apply. 
  • How do I contact the health care navigator?
    You can contact the health care navigator by calling (813) 870-4299, emailing angelinenavigator@baycare.org or visiting the Angeline Welcome Center. The health care navigator is available online Monday through Friday, 8am to 4:30pm. Messages received after hours or on weekends will be processed the next business day.
  • Will the health care navigator be available on-site?
    Yes, the health care navigator will be available at the Angeline Welcome Center two half-days every week. For more information, call (813) 870-4299 or email angelinenavigator@baycare.org.
  • How can the health care navigator help me?

    The health care navigator can help you connect with BayCare services and resources, including:  

    • Finding the right primary care provider or specialist based on your needs and coordinating imaging and lab appointments
    • Receiving in-person blood pressure and diabetes screenings
    • Understanding symptoms and where to go for care
    • Getting answers to general health and wellness questions
    • Receiving follow-up support to help you connect with the services you need
    • Using the BayCare patient portal, BayCareAnywhere®, the BayCare app and other health care tools 

    As the program grows, additional services may be added.
  • Can the health care navigator schedule appointments for me?
    Yes, the health care navigator can help you coordinate and schedule appointments.
  • What does priority access to BayCare services mean?
    Priority Access means you have a health care navigator who can help connect you to BayCare services, including imaging, lab testing, primary care and specialist appointments. The navigator can also help you understand your care options and get the services you need.
  • How is this different from having a primary care provider?
    A primary care provider diagnoses, treats and manages your health care needs. The health care navigator doesn’t replace your provider. Instead, the navigator can help connect you with BayCare services, answer questions, coordinate care, assist with online tools and make the health care experience easier to navigate.
  • Can the health care navigator help me use the BayCare portal and other digital tools?
    Yes, the health care navigator can help you feel more comfortable using BayCare's online tools, including the BayCare patient portal, BayCareAnywhere and the BayCare app. These tools can help you manage appointments, access health information and get care when and where it's most convenient for you.
  • What health and wellness services are available through the program?

    The health care navigator helps you learn about and connect with services such as: 

    • Primary care and specialist services 
    • Annual wellness visits and preventive care 
    • Blood pressure and diabetes screenings 
    • Cancer screenings
    • Women's and men's preventive health services
    • Weight management resources
    • Nutrition resources
    • Behavioral health resources
    • Lab services
    • Urgent care and emergency care resources
    • BayCare Kids programs and community wellness programs

     Additional services may be added as the program grows.

  • Will there be screenings and educational events in the community?
    Yes, BayCare plans to provide on-site health screenings, including blood pressure and diabetes screenings. The program may also offer community events focused on health education, wellness, technology support and preventive health.
  • What if I'm healthy and don't need medical care?
    The BayCare health care navigator program isn't just for when you're sick. You can use the program to learn about preventive screenings, wellness resources, healthy lifestyle programs, nutrition resources, exercise opportunities and online tools that support your long-term health.

Advanced Care for Adults and Children

Angeline residents have access to advanced care close to home at BayCare Hospital Wesley Chapel and BayCare St. Joseph's Hospital-North. Both hospitals offer inpatient and outpatient services and care for a wide range of health needs. Services include emergency care for adults and children, heart and vascular care, orthopedic care and urologic and gynecologic surgery.
