Identifying Health Needs, Finding Solutions - Winter Haven Hospital

As a leading community-based health system in the greater Tampa Bay area, BayCare Health System is committed to meeting the wide-ranging health needs of our neighbors in every community we serve. As part of that commitment, BayCare conducted a comprehensive Community Health Needs Assessment (CHNA) every three years to identify the most significant health needs across our region and develop strategies for addressing them. A collaborative process is used to develop the assessments (and implementation plans) integrating extensive data analysis, community member feedback and in working with community organizations that share our commitment.

For each facility's community, the following are available:

What you will find when reviewing the 2015 Community Health Needs Assessment: 

  • Information about how the assessment was conducted (i.e., purpose, area assessed, data collected, etc.) is available in the first five sections of the report (Introduction, Community Definition, Consultant Qualifications, Project Mission & Objectives, and Methodology)
  • An executive summary of top findings related to health needs identified in the community can be found in the Key Community Health Priorities section. 
  • A summary of supporting documentation relevant to the top findings is available in the final two sections (Secondary Data Profile and Key Stakeholder Interviews).
  • All data that was used in the needs assessment is available for your review in Appendices (Appendix A: Community Resource Inventory, Appendix B: Community Secondary Data Profile).
