Hospice Care

Hospice is a special concept of care designed to provide comfort and support to patients and their families. Hospice staff and volunteers offer a specialized knowledge of medical care, including pain management. The goal of hospice care is to improve the quality of life by offering comfort and dignity.

Hospice care is primarily provided in the home, where patients have the loving support of their family and friends. However, most hospice providers define "home" as any place that the patient chooses to reside.

Many hospice providers offer the following services: 

• 24-hour on-call nursing
• home health aides
• patient care education and training for caregivers
• 24-hour on-call counseling
• trained volunteer support
• spiritual support during illness
• respite services for caregivers
• bereavement and spiritual counseling for loved ones for up to a year after the patient's death
• specialized programs for children
• professional caregivers for patients with a fragile caregiver, or no caregiver at all

Pastoral Care

The spiritual, emotional and religious needs of our patients, families and team members are important to us. The Pastoral Care teams in our hospitals represent various religions and will assist you in contacting your own place of worship.

In addition to providing spiritual support to patients and families of all faith traditions, our Pastoral Care team offers: 

• Counseling regarding Advanced Directives, which are specific instructions prepared to direct medical care for persons if they become unable to do so for themselves in the future.
• Help in finding clergy for any denomination.
• Support in times of psycho spiritual crisis for our patients and their families. 
• Eucharistic ministers to provide Holy Communion to those wishing to receive. 
• A quiet room on the patient floor for private conversations, meditation, prayer and reflection throughout the hospital stay.

Social Work Service

Social workers are available to help with your discharge from the hospital. Social workers can help you plan for ongoing medical needs by giving referrals to home health, skilled nursing or rehabilitation services, and offer help with financial concerns, transportation and other community resources. Your social worker is a valuable resource for information that may be useful to you during your recovery.
