All mortalities are reviewed for areas of opportunity.  Compliance to Evidence Based Medical Practice is measured and reported. Patients are seen daily by a multidisciplinary care team to ensure timely, appropriate care is given for their disease process, including admission to Hospice if that is best for the patient. We review all Heart Failure cases using American College of Cardiology, Society of Thoracic Surgery, and Agency for Healthcare and Research and Quality criteria for opportunities to prevent complications and improve care. All mortalities are reviewed for areas of opportunity.  Compliance to Evidence Based Medical Practice is measured and reported. Patients are seen daily by a multidisciplinary care team to ensure timely, appropriate care is given for their disease process, including admission to Hospice if that is best for the patient. Patients are referred to the post-acute center on discharge, and set up with ARNP home care, pulmonary rehab, and/or the heart function clinic for care as appropriate. We continue to partner with high performing skilled nursing facilities to ensure patients receive early, appropriate care for worsening symptoms. Multidisciplinary COPD workgroup has implemented checklist for patient education.  Physician review of all COPD deaths implemented to look for any additional care improvement opportunities.
