Chronic Complex Clinic (Tampa)

The Daniel J. Plasencia, MD Children’s Chronic Complex Clinic and the Adult Chronic Complex Clinic at St. Joseph’s Hospital work together to create a patient-centered medical home (PCMH) where children and adults receive primary care services in a friendly and professional atmosphere. We specialize in caring for children and adults with a variety of complex medical conditions including premature birth, congenital disorders, cerebral palsy, heart conditions and traumatic brain injuries. Our dedicated multidisciplinary team works with patients and caregivers to provide a family-centered, compassionate plan of care. In addition to offering education on health care self-management, we organize and coordinate all patient care activities with specialists and facilitate durable medical equipment and home health care services.

As a PCMH, we provide consistent care and comprehensive health services in one place, including primary care, care coordination of specialty care services, continuity of care during hospitalizations, and also help bridge the gap between the highly specialized pediatric and adult medical services that these patients need throughout their lifetime, giving them a continuum of care. As children enter adulthood, patients can move seamlessly from pediatric to adult care, all in the same location.

The Chronic Complex Clinic is located in the St. Joseph’s Hospital Medical Arts Building. The Medical Arts Building is adjacent to the main hospital. Once in the Medical Arts Building, take Elevator A to the 3rd floor. Follow the red tiles on the floor until reaching the Chronic Complex Clinic.
